Under Texas law, clients have the right to access their health care records and contact state agencies with concerns about their care.
Accessing Your Health Care Records
Requests for copies of health care records or written summaries must be submitted to our office in writing. All requests are handled in compliance with state and federal regulations. Processing fees may apply. Our office staff is available to help with questions about submitting a request.

Submitting a Consumer Complaint
